Thank you for referring someone you know to Melody Care!
In order to process both of your gifts we just need a few details from you using the form below and we will process this as soon as the person you referred has taken out our service and paid their deposit.
Terms and Conditions:
1. It is required that you fill in the above form and include the details of the person you have referred.
2. The person you refer must have taken out a care package with us, signed an agreement and paid their deposit.
3. Once this form is completed and the person referred has paid their deposit, we will issue the voucher to both of you via email within 30 days.
4. You can choose to have either a £25 Marks and Spencer voucher or a £25 donation to a charity of your choice.
5. There is no cash alternative
